New Client Information

 

Appointment/Cancellation Policies:

  • A credit card number on file is needed to hold your appointment.  Appointments may be make online by clicking 'Schedule Appointment' from any page on the website. 

  • Accepted forms of payment are cash, credit card or gift certificate.

  • If you need to cancel or reschedule your appointment, I kindly ask for at least 24 hours notice.  You will receive a reminder 48 hours prior to your appointment.  

  • If you cancel with less than 24 hours notice, your card on file will be charged 50% of the session fee.  

  • Cancellations within four (4) hours of your appointment, or no shows, will be charged 100% of your scheduled session.  These charges are based on the full, non- discounted rate.  For extenuating circumstances, you may request an exception, which will be granted or denied at my discretion. 

  • After scheduling your appointment you will receive a confirmation with a link for the Intake Form.  If for some reason you can not access them, the Intake Form link is also below.  These must be filled out, signed and returned within 48 hours of your appointment.

  • Please arrive 5-10 minutes early for any appointment in order to get the full session time you have scheduled.
